This form is to authorize NGBC to obtain necessary medical attention in case of sickness or injury.

Each spring on Mothers Day, one college student, graduate student or high school senior will be chosen from applying members of NGBC.

This scholarship was set up and funded in January 2023. Each year on Senior Day two recipients, one male and one female, will each receive a $ 500 scholarship.

The NGBC H.I.S Women's Ministry Scholarship Program is offered to a student who actively attends NGBC.

The NGBC Men's Ministry Scholarship Program is offered to a student who actively attends NGBC.

To be completed for a group requesting approval for a Mission Trip/Project.

This form is filled out by anyone using the church facility. It lists use policies and fees.

This form is filled out by anyone wishing to have their wedding in our church.

This form is to be completed to request employment at North Greenwood Baptist Church.

This form is filled out by any parent wishing to send their child to His Kids Child Care Center. There is a $ 25.00 fee to be returned with this form to hold your spot.

This form is filled out by anyone wishing to have their bank account drafted for their offering to the church.  A voided check must be sent with this form to Patricia Ethridge at the church to be effective.
